I am writing to inform you that I will be retiring from medical practice effective March 20th, 2024. It has been an honor and privilege to serve as your child's pediatrician over the years, and I am deeply grateful for the trust and confidence you have placed in me. To ensure that your medical records will be available to you I have arranged for them to be maintained by Northwell Health as of April 1st, 2024.

To schedule an appointment with any of these practices please call the location most convenient for you. If you wish to discontinue your care with Northwell Health Physician Partners, they will forward your medical records to the provider of your choice. You will need to sign a release of medical record authorization form. Please call (833) 321-3388 or email mymedicalrecords@northwell.edu to request a copy of the authorization form.
